Filing through the Board's portal
New York’s Workers’ Compensation Board provides an online path to submit the Employee Claim (Form C-3). Filing online means your claim reaches the Board directly and you get confirmation of submission, rather than waiting on the mail.
You will need the basics ready: your information, your employer’s name and address, the date and description of the injury, and the body parts affected. Having your written notice and any medical paperwork on hand makes the form go faster.
After you submit
Once filed, your claim is indexed and assigned a case number, and the employer’s insurer is notified and must respond. You can follow the case in the Board’s online system, where documents and decisions in your claim appear over time.
Watch for the carrier’s response – acceptance and the start of payments, or a dispute. Either way, the case number is your key to tracking what happens next.
Keeping your account in order
Save your confirmation and case number, and check the online file periodically so you are not surprised by a notice or a deadline. If a document you expect is missing, that is worth following up on.
Online filing is a convenience, not a substitute for good records. Keep your own copies of everything you submit, so you can always show what was filed and when.
