Filing your claim, step by step
1
Report to your employer
Notify your employer in writing within 30 days of the injury – this protects your right to benefits.
2
Get authorized medical care
See a provider authorized by the NY Workers’ Comp Board; the provider files the medical report on your condition.
3
File Form C-3
Submit your employee claim to the Board online, by mail or email, or in person – not by telephone.
4
The carrier responds
The insurer must begin payments or formally dispute the claim, generally within about 18 days of learning of the injury.
